IMPORTANT - Coronavirus Statement & Guidelines for Keeping Sick Children Home

Health and Wellness Information from your School Nurse

Parents/Guardians and students, as we all work together to ensure we have a successful Back to School experience, please review the following:

  1. Students with parent assistance (elementary and middle) will complete a Wellness Screen daily at home.  High School students will complete at school. Staff will also complete a Wellness Screen daily. 
  2. Students that are feeling ill, should stay home and complete assignments virtually as able.
  3. Masks or face covering are required for staff and students (3rd grade and up) at certain times throughout the day when it may be difficult to enforce social distancing. Students that don’t have a mask when arriving to school, will be offered one at school.
  4. Students and staff with a medical condition that may be impacted by wearing a mask are not required to wear one.  Face shields will be available for staff
  5. Students that develop symptoms at school may be kept in his/her class or escorted to a designated area until a parent, guardian or designated adult can pick them up. *This information has been reviewed by the Health Department 
  6. If a student is sent home with symptoms, it is likely medical follow-up will be recommended and a note from provider needed prior to his/her return.
  7. In order to assist with communication between school, medical provider, parent/guardian and school nurse, you may fax or e-mail information to child’s school or the nurse. *See contact information on county website.

New students entering PreK or K need proof of a dental exam and a health check physical exam prior to starting school in August. It is recommended students entering grade 2 and now grade 7 show proof of a dental exam and a health check physical exam prior to school starting in August.  

Also there are new requirements for vaccines for 7th and 12th grade students.

Information for 7th & 12th graders in Hampshire County Schools

Parents this is a reminder. State law and WV Board of Education require all 6th and 11th graders to receive vaccine updates prior to entering 7th and 12th grade.

All students entering the 7th and 12th graders must show proof of these vaccines prior to the first day of school in August 2020. 

Hampshire County Health Department  will be visiting the high school and both middle schools on March 10th and again on April 21st to offer these vaccines to students with parent permission.  Permission forms are available below or from the School Nurse or the Health Dept. 

Vaccine updates can be obtained from your child’s Dr. or from the Health Department. The Health Department vaccinates every Monday from 8:30am – 3:30 pm. On the first and third Monday of the month beginning in April, the Health Department will offer vaccines from 8:30am – 5:30pm.   Ph: (304) 496-9640  

  • NEW - Medical Information Release Form (FERPA) - may be typed, then printed & signed
  • Prescription Medication Information
  • Prescription Medication Authorization Form
  • Hampshire High Over-the-Counter Medication Information and Authorization Form regarding Ibuprofen (Advil), acetaminophen (Tylenol) and antacids (Tums)
  • Chronic Health Condition Statement  - Print this form and have it completed by a physician and faxed to the Attendance Director to alert the district to possible chronic conditions that may impact student attendance.
