Proof of Attendance for Driver's Licenses

Students between the ages of 15-18 may obtain a Proof of Enrollment Form in the Superintendent's Office - Contact the Office of the Superintendent for your Proof of Enrollment - 304-822-3528; ext. 120.

In order to be eligible for a Proof of Enrollment Form a student:
  1. Cannot have more than 10 consecutive or 15 total days unexcused absences during a school year. 
  2. Must maintain Satisfactory Academic Progress as defined in WVDE Policy 4150. 
  3. Cannot have any Safe School Violations.Suspensions pursuant to WV Code

WV Read Aloud Volunteer Training - Oct 2 at Hampshire Co. Health Dept. @ 10 AM - Register Now There will be a “Read Aloud” training on Tues, Oct 2, 2018, at the Hampshire County Health Dept. from 10:00 am.- Noon for volunteer readers in our elementary schools. New Procedure to Become a School Volunteer Volunteers are needed at all schools. Complete an application which requires approval of the school principal. Then apply for your background check. Upon your background clearance, you can be approved as a volunteer. Click for more information...
