Please sign up for the Back-to-School PEIA Wellness Screening using the online form on this website or go to it directly on the Internet at PEIA Wellness Sign-up Form.
The screening is open to all PEIA insured employees at the HHS Library from 6:30 - 8:15 am on Friday, August 20, 2010 and includes routine cholesterol and blood glucose testing. Other blood tests are available for minimal fees to insureds. A twelve (12) hour fast is recommended for accurate blood tests.
Slots are available for approximately 6 persons to be tested every 15 minutes. Once you have completed the form indicating your preferred times, you will be notified via email or phone of your actual appointment time slot based on availability. Participation in this screening may qualify insureds for a PEIA discount beginning in 2012. This might be the last chance to qualify to get the PEIA discount effective in 2012 as the testing will only be given every other year in the future.
