Posted on: September 25, 2018[ARCHIVED] Flu Shots Will Be Available for HHS. RMS & CBMS Stidents
Parents: The Health Dept. will be visiting Hampshire High School and both middle schools to offer the Flu vaccine to students in those schools. They will be at Capon Bridge Middle on 10/4 and at HHS and RMS on 10/11.
If interested, please do one of the following:
Have you child pick up a consent form from the School Nurse
Call your child’s school and request consent form, can be sent with student or faxed
Pick up consent form from the Health Dept.
The completed consent form needs to be turned into the School Nurse no later the Friday, September 28th.
